Visuoperceptual Abilities Questionnaire

Hi all,

I'm an AP working in a memory service and was wondering if anyone could point me in the right direction for a validated self-report / informant questionnaire that covers visuospatial / perceptual abilities? This will primarily be for assessments that we conduct over the phone.

I'm working in neuropsychology and I can't think of a self-report measure that would do that (admittedly I'm paeds, but we do assess up to age 25). The best I can think of is a good clinical interview that elicits examples of tasks in which I'd expect to see visuo-perceptual errors occurring. I'll watch this thread with interest to see if anyone else knows of anything.
